Submission of application form |
|
Application on prescribed form should be submitted to Principal, College of nursing, Bombay Hospital, Indore before the last date along with the following enclosures: |
| - Bank draft for Rs.500/- in favor of Bombay Hospital (for purchasing prospectus). |
| - Filled in application form. |
| - Passport size attested photographs (3) and attested photocopy of 10th and 12th mark sheets. |
| - At the time of interview original mark sheets of 10th &12th will be required. In case, result is not declared must |
mention in the application, but have to produce the mark sheet at the time of interview. |
| - If SC/ST/OBC, attested certificate of the same. |
| - Medical fitness certificate from the authorized Govt. Medical Officer. |
